Please completely fill in the information requested below and return with registration fee(s) to the AWANA Office Desk.  Registration fee for each additional child after the first child is $15.  Make checks payable to:  Bethel Baptist Church AWANA.  Complete a BBC Participant Form for each child and give it to Barbara Snyder, Director of Children’s Ministry.  Free notary service for this form is available in the church office.

Cubbies MUST be age 3 by Sept. 1, 2006 and fully potty trained – Please, No Exceptions.
